Mount Vernon School District families and student-athletes use our online forms and data management service called FinalForms to complete and sign athletic participation for students. The information is saved from season-to-season and year-to-year, meaning you do not need to enter the same information more than once. FinalForms also pre-populates a student's information wherever possible, saving you time. All active students are added to FinalForms through Skyward so parents/students will already have an account as long as you are active and have at least one parent's email address in Skyward.
ALL families and student-athletes are required to use FinalForms, and to have an updated physical to register for sports. Registration forms must be completed PRIOR to the first practice to be eligible to participate. You can review your data at any time to verify it is current. You will be required to sign your forms once a year and after any updates.

All student-athletes are required to have a valid sports physical (good for two years) prior to their first practice. If you plan to see your personal doctor, please make your appointment right away as appointments can be hard to get. You can present this Physical Clearance Form for your doctor to complete.

Students Who Don’t Attend MVHS Full-Time
Students are required to complete a WIAA contract if they do not attend the school full-time. These contracts must be turned in before the first grade check of each season to remain eligible for competition.
Running Start
If you are enrolled in Running Start, complete the Running Start and WIAA Eligibility Contract with your class schedule and return it to the athletic office.
Off-Campus Classes
If you are enrolled in any off-campus classes, complete the Alternative Education and WIAA Eligibility contract with your class schedule and return to the athletic office.
Home School
If you are home-schooled, complete the Home-Based Education and WIAA Eligibility contract with your class schedule and return it to the athletic office.
Grades
Grade checks are run throughout each sports season. Failure to provide grades may make you ineligible for competition.
If you attend any other school for ANY classes, you must provide the Athletic Office with your grades from that school. Please email grades to Jenni Stahlecker at jstahlecker@mvsd320.org.
Notice to middle school parents & students who attend High School Athletic events: Middle and high school sports are vital to the growth and development of all students, and participation in sports helps build self-esteem and creates memories that last a lifetime. We want all students and families to have positive experiences while attending sporting events at Mount Vernon High School.
Middle school students must pay the student price to enter high school events. Middle school students with ASB are not admitted for free. Students will be expected to demonstrate positive fan behavior and sportsmanship. This includes but is not limited to:
- Sitting in the bleachers for the duration of the game.
- Using appropriate language and treating others with respect
- Complying with direction from stadium and/or high school officials
- School rules and behavior expectations apply to district athletic events and venues
Students who do not meet the expectations of either the stadium or high school staff may be asked to leave the facility, and staff will attempt to contact parents/guardians. Students who violate school rules could potentially receive school discipline and be No-Trespassed by the Mount Vernon Police Department.
High School events, such as football games at MVHS, are meant to be enjoyed by the entire community. Please attend these events with your student whenever possible, and when you cannot, review the expectations for their behavior ahead of time.
High School Sports Tickets
The following sports charge for tickets: basketball, football, soccer, volleyball, and wrestling. Regular season prices are shown in the chart below. Post-season ticket prices are generally higher than regular-season ticket prices and are shown in GoFan for each separate post-season event. At MVHS home games, we accept cash, checks, and all major credit cards for admissions.
Adults/HS Students without ASB Card - $7.00
Visiting HS Students with ASB Card - $5.00
Elementary/Middle School Students (K-8) - $5.00
Military ID, Senior Citizens (62+) - $5.00
Home Student with ASB Card / Pre-K - Free
